ABSTRACT Objective To describe the prevalence of inadequate mineral intake and associated factors with calcium, iron, zinc, magnesium, phosphorus, and sodium intakes in individuals aged 15-24.9 years. Methods We analyzed 476 individuals from the Brazilian Study of Nutrition and Health, stratified into two age groups (adolescents aged 15-18.9 years and young adults aged 19-24.9 years). Mineral intake was obtained from two 24-hour Dietary Recalls. The values of the Estimated Average Requirement and the Tolerable Upper Intake Levels were considered to calculate the prevalence of inadequacy. Multiple logistic regression was used to determine associated factors with mineral intake. Results Calcium and magnesium had the highest prevalence of inadequacy (>83%) in both sexes and age groups. Sodium intake was above Tolerable Upper Intake Levels for the majority of the population studied (>68%). The intake of all minerals was different between the sexes for the two age groups (p<0.01), and it was not different between age groups (p>0.05). The associated factors with mineral intake were sex (calcium, iron, phosphorus, and sodium), age group (calcium, magnesium, phosphorus, and sodium), and physical activity (calcium, iron, and magnesium), followed by socioeconomic level (zinc and sodium) and body weight status (iron and sodium). Conclusion The expressive portion of the studied population is at nutritional risk for calcium, magnesium, and sodium. Such data can contribute to the national public policy revision that is related to micronutrient intake and the adoption of healthier habits by adolescents and young adults.


RESUMO Objetivo Descrever as prevalências de inadequação e fatores associados à ingestão de cálcio, ferro, zinco, magnésio, fósforo e sódio em indivíduos dos 15 aos 24,9 anos. Métodos Foram analisados 476 indivíduos do Estudo Brasileiro de Nutrição e Saúde, estratificados em dois grupos etários (adolescentes de 15-18,9 anos e adultos jovens de 19-24,9 anos). A ingestão de minerais foi obtida por meio de dois recordatórios de 24hs. Os valores de Requerimento Médio Estimado e do Limite Superior Tolerável de Ingestão foram considerados para calcular as prevalências de inadequação. Regressão logística múltipla foi utilizada para determinar os fatores associados à ingestão de minerais. Resultados Cálcio e magnésio tiveram elevadas prevalências de inadequação (>83%) em pacientes de ambos os sexos e grupos etários. A ingestão de sódio foi acima Limite Superior Tolerável de Ingestão para a maioria da população estudada (>68%). O consumo de todos os minerais foi diferente entre os sexos para os dois grupos etários (p<0,01) e não foi diferente entre os grupos etários (p>0,05). Os fatores associados à ingestão dos minerais foram sexo (cálcio, ferro, fósforo e sódio), grupo etário (cálcio, magnésio, fósforo e sódio) e atividade física (cálcio, ferro e magnésio), seguidos por nível socioeconômico (zinco e sódio) e estado de peso corporal (ferro e sódio). Conclusão Expressiva parcela da população estudada encontra-se em risco nutricional para cálcio, magnésio e sódio. Esses dados podem contribuir para a revisão de políticas públicas nacionais que se relacionam à ingestão de micronutrientes e à adoção de hábitos mais saudáveis pelos adolescentes e adultos jovens.

ABSTRACT

Introducción: conocer la deficiencia de micronutrientes en la ingesta es imprescindible para desarrollar estrategias de intervención y para evaluar programas destinados a mejorar el estado nutricional. Objetivos: determinar el consumo y la inadecuación de la ingesta de hierro, calcio, zinc, ácido fólico y vitaminas A, B12, C,D y E en la dieta de la población argentina. Materiales y métodos: la muestra fue de 1.266 individuos (población urbana 15-65 años), estratificada por región, edad, sexo y nivel socioeconómico (NSE). Los datos de ingesta fueron recolectados con dos recordatorios de 24 horas (R24). Resultados: el riesgo relativo (RR) de inadecuación de ingesta de calcio, ácido fólico y vitamina B12 fue mayor para las mujeres. El consumo y la inadecuación de hierro mostraron diferencias significativas por sexo (0,3% hombres, 11% mujeres). La inadecuación de vitamina D fue casi del 100% para toda la muestra. Todos los grupos etarios presentaron alta inadecuación de calcio y vitaminas C, A y D. Adolescentes, adultos mayores de 50 años y población de NSE bajo presentaron mayor RR de inadecuación de calcio (1,18; 1,21 y 1,17 respectivamente). Se observó una disminución significativa de la media de consumo de calcio (mg) (856,4; 770,5; 745,3), vitaminas A (mcg) (643,6; 601,8; 536,2), C (mg) (54,2; 52,3; 44,8) y D (mcg) (3,7; 3,7; 3,2) al disminuir el NSE. La inadecuación en calcio aumentó significativamente con el índice de masa corporal. Conclusiones: mejorar la ingesta de micronutrientes a través de políticas públicas sobre educación alimentaria y desarrollo científico es una prioridad de salud pública


Introduction: knowing the deficiency of micronutrients intake is essential to develop intervention strategies and to evaluate programs aimed at improving nutritional status. Objectives: to determine the consumption and inadequacy of the intake of iron, calcium, zinc, folic acid and vitamins A, B12, C, D and E, in the diet of the Argentine population. Materials and methods: the sample consisted of 1,266 individuals (urban population 15-65 years old), stratified by region, age, sex and socioeconomic level (SEL). Intake data was collected with two 24-hour reminders (R24). Results: the relative risk (RR) of inadequate intake of calcium, folic acid, and vitamin B12 was higher for women. Iron consumption and inadequacy showed significant differences by sex (0.3% men, 11% women). Vitamin D inadequacy was almost 100% for the entire sample. All age groups presented high prevalence of inadequacy in calcium and vitamins C, A and D. Adolescents, adults over 50 years and the low SEL population presented a higher RR of calcium inadequacy (1.18, 1.21 and 1.17, respectively). A significant decrease in the average consumption of calcium (mg) (856.4, 770.5, 745.3), vitamins A (mcg) (643.6, 601.8, 536.2), C (mg) (54.2, 52.3, 44.8) and D (mcg) (3.7, 3.7, 3.2) was observed as the SEL decreased. Calcium inadequacy increased significantly with the body mass index. Conclusions: improving the intake of deficient micronutrients through public policies based on food education and scientific development is a public health priority

Diversos estudios epidemiológicos y de intervención apuntan a una asociación fuerte y consistente entre la ingesta excesiva de azúcares y el riesgo de desarrollar caries dentales y enfermedades crónicas no transmisibles. El objetivo fue describir los 10 principales alimentos que contribuyen a la ingesta de azúcares añadidos en muestras representativas de poblaciones urbanas de ocho países latinoamericanos, y considerar diferencias por país, sexo, nivel socioeconómico (NSE) y grupo de edad. Se realizó una encuesta transversal multinacional en hogares de Argentina, Brasil, Chile, Colombia, Costa Rica, Ecuador, Perú y Venezuela (N=9218; 15­65 años). La contribución porcentual a la ingesta total del nutriente sirvió para identificar los alimentos aportadores. Los datos se estratificaron por país, sexo, NSE y grupo de edad y se compararon los gramos por día. Excepto en Perú y en el grupo de 50 a 65 años, los refrescos figuraron como el principal contribuyente a la ingesta de azúcares añadidos. Las bebidas caseras y las industrializadas también destacaron. Argentina fue el país con mayor ingesta de azúcares añadidos aportados por bebidas industrializadas, y Chile el país con menor frecuencia de bebidas como fuentes. Entre los 10 contribuyentes, la ingesta de azúcares añadidos fue mayor en el sexo masculino, excepto para sacarosa y pasteles dulces. Mayores ingestas de azúcares provenientes del jugo de fruta natural y sacarosa fueron observadas en el NSE más bajo. Las mayores y menores ingestas en los grupos de edad cambian de acuerdo con la fuente alimentaria. En conclusión, las bebidas azucaradas fueron los principales contribuyentes a la ingesta de azúcares añadidos, y la ingesta varió según la ubicación geográfica y los factores sociodemográficos(AU)


This study aimed to describe the top 10 foods that contribute to added sugars intake in representative samples of urban populations in eight Latin American countries, and consider differences by country, sex, socioeconomic level (SEL), and age group. A household-based, multinational, cross-sectional survey was conducted in Argentina, Brazil, Chile, Colombia, Costa Rica, Ecuador, Peru, and Venezuela (N=9218; 15­65 years). Contributors to added sugars were identified and listed based on the percentage of contribution to total intake of this nutrient. Data were stratified by country, sex, SEL, and age group, and the grams per day compared. Except in Peru and in the 50 to 65 years age group, soft drinks were the leading contributor to added sugar intake. Homemade beverages together industrialized beverages have been a prominent position on ranking. In general, the highest intake of added sugars by industrialized beverages was from Argentina, and the lowest frequency of beverages as sources of added sugars was observed in Chile. Among the top 10 contributors, male sex had highest added sugar intake, except for sucrose and sweet cakes. Higher intakes of sugars from natural fruit juice and sucrose were observed in the lower SEL. The highest and lowest intakes in the age groups change according to the food source. In conclusion, sugar-sweetened beverages were main contributors to added sugar intake, and the intake vary with geographical location and sociodemographic factors(AU)

SUMMARY OBJECTIVE We investigated the associations between objectively assessed sedentary behavior (SB) and moderate-to-vigorous physical activity (MVPA) and body composition variables among a representative sample of Brazilian adults. METHODS Using an accelerometer, SB and MVPA were monitored for at least 5 days in 524 participants (261 men; age, 18-65 years). Each minute epoch was classified as sedentary or spent in light, moderate, or vigorous physical activity (LPA, MPA, and VPA, respectively). The measured body composition variables included abdominal perimeter (AP) and neck circumference (NC). RESULTS Men accumulated significantly more min/day of MPA (37.82 versus 27.28), VPA (1.10 versus 0.31), MVPA (39.02 versus 27.61), and steps/day (14,978 versus 13,443) than women (p<.001). In men, MPA, VPA, MVPA, and steps/day were negatively associated with AP (p<.05) independently of SB. Only VPA was significantly associated with NC (β= 0.113; p=.002). In women, only SB was significantly associated with AP (β= 0.003; p=.031). There were no significant associations between physical activity intensities and body composition in women. CONCLUSIONS Our findings on the unequal association of physical activity with body composition variables between sexes can help inform future intervention strategies in Brasil.


RESUMO OBJETIVO Investigar a associação do comportamento sedentário (CS) e da atividade física de moderada a vigorosa (AFMV), avaliados objetivamente, com variáveis de composição corporal em uma amostra de adultos brasileiros. MÉTODOS CS e AFMV foram monitorados por meio de acelerômetros no mínimo por cinco dias, em 524 participantes (231 homens; 18-65 anos). Cada período de epoch de um minuto foi classificado como sedentário, atividade física leve (AFL), moderada (AFM) ou vigorosa (AFV). As variáveis de composição corporal medidas foram: perímetro abdominal (PA) e circunferência do pescoço (CP). RESULTADOS Os homens acumularam significativamente (p<0,001) mais min/dia em AFM (37,82 versus 27,28), AFV (1,10 versus 0,31), AFMV (39,02 versus 27,61), e passos/dia (14.978 versus 13.443) do que as mulheres. Nos homens, AFM, AFV, AFMV e passos/dia associaram-se (p<0,05) negativamente com PA, independentemente do CS. Somente AFV associou-se significativamente (β= -0,113; p=0,002) com CP. Já nas mulheres, apenas CS associou-se significativamente com o PA (β= 0,003; p=0,031). Não houve associações significativas entre as intensidades de atividade física com a composição corporal nas mulheres. CONCLUSÕES Nossos achados sobre a associação desigual da atividade física com composição corporal entre os sexos aumentam a base de evidências e podem ajudar a informar futuras estratégias de intervenção no Brasil.

ABSTRACT

Objetivo: identificar fatores associados à prevalência dos diferentes tipos de dificuldades alimentares em crianças com idade entre 0 e 10 anos. Métodos: trata-se de um estudo observacional retrospectivo com dados de crianças de 0 a 10 anos diagnosticadas com alguma dificuldade alimentar, atendidas em um centro de referência localizado no município de São Paulo, SP, Brasil. A presença da dificuldade alimentar foi analisada de acordo com o diagnóstico multidisciplinar, o aparecimento da queixa, os dados sobre a gestação e a amamentação, o estilo parental, a presença de causas orgânicas e as complicações associadas. Os dados foram tabulados no programa Microsoft Excel e as análises estatísticas foram realizadas no programa IBM SPSS Statistics 21 através de frequências absolutas e relativas, média, desvio padrão, teste do qui-quadrado e Kruskall-Wallis com o teste de post-hoc de Dunn. Resultados: foram avaliados 166 pacientes com um predomínio do sexo masculino, em idade pré-escolar e nascidos a termo. O diagnóstico mais prevalente foi de seletividade alimentar, sem doenças orgânicas ou reações associadas. Os diagnósticos de dificuldade alimentar apresentaram associação estatisticamente significativa com idade, idade gestacional de nascimento, idade e fase da alimentação de aparecimento da queixa. Conclusões: os fatores que apresentaram associação com os diferentes tipos de dificuldade alimentar foram a idade, a idade gestacional de nascimento e a idade e fase da alimentação de aparecimento da queixa. O presente estudo permite iniciar um direcionamento mais específico de ações de orientação e de prevenção de dificuldade alimentar. Dessa forma, destaca-se a importância da realização de estudos desse cunho.


Aims: To identify factors associated with the prevalence of different types of feeding difficulties in children aged 0 to 10 years. Methods: This is a retrospective observational study with data from children aged 0-10 years diagnosed with some feeding difficulty, attended at a reference center located in the city of São Paulo, Brazil. The presence of feeding difficulty was analyzed according to the multidisciplinary diagnosis, appearing of the complaint, information about pregnancy and breastfeeding, parenting style, presence of organic causes and associated complications. Data were tabulated using Microsoft Excel software and statistical analyzes were performed using IBM SPSS Statistics 21 using absolute and relative frequencies, mean, standard deviation, chi-square test and Kruskall-Wallis test associated with Dunn post-hoc test. Results: 166 patients were analysed, with male, preschool age and full-term predominance. The most prevalent diagnosis was picky eating, with no associated organic diseases or reactions. Diagnoses of feeding difficulty were statistically significant associated with age, gestational age at birth, age and feeding phase of onset of the complaint. Conclusions: The factors that were associated with the different types of feeding difficulty were age, gestational age at birth, age and feeding phase of emergence of the complaint. The present study allows us to initiate a more specific direction of orientation actions and prevention of feeding difficulties, thus highlighting the importance of conducting studies of this nature.

INTRODUCTION: With the increase of childhood obesity, early intervention in nutrition education becomes necessary. Therefore, during the school period, the school becomes the best place for implementation of a nutritional education program. OBJECTIVE: The current study was carried out in a public and in a private school, with the objective of evaluating and comparing the results of the "Nutriamigos®" Program on food and nutrition knowledge of children with different socioeconomic levels, sex, age and Body Mass Index (BMI). METHODS: Comparative longitudinal study, before and after educational intervention. To evaluate the intervention, a public (school 1) and a private school (school 2) were selected. This was a random, convenience sample comparing students from different social and economic levels, consisting of 242 (school 1) and 99 children (school 2), respectively, totaling 341 children, from 6 to 10 years of age. The Body Mass Index was used to measure nutritional status. Knowledge about food and nutrition was evaluated through a questionnaire, applied before and after intervention. The nutritional intervention consisted of 12 classes of 50 minutes each. RESULTS: The improvement in knowledge on food and nutrition between the pre and post intervention stages was statistically significant. CONCLUSION: The "Nutriamigos®" Program reached its goal, proving that it is effective in public and private schools and should not be different for boys or girls, or for children with normal or excessive weight.


INTRODUÇÃO: Com o aumento da obesidade infantil, torna-se necessária intervenção educacional nutricional desde a infância. Durante o período escolar, a escola se torna, portanto, o melhor local para a implementação de um programa de educação nutricional. OBJETIVO: O presente estudo foi realizado em uma escola pública e uma particular, com o objetivo de avaliar e comparar o resultado da intervenção do Programa "Nutriamigos®" no conhecimento sobre alimentação e nutrição, de crianças de diferentes níveis socioeconômicos, sexo, idade e Índice de Massa Corporal (IMC) MÉTODO: Estudo longitudinal comparativo, antes e após intervenção educacional. Para a avaliação da intervenção foram selecionadas uma escola pública (escola 1) e uma particular (escola 2). Esta foi uma amostra de conveniência aleatória, comparando alunos de diferentes níveis sociais e econômicos, consistindo de 242 (escola 1) e 99 crianças (escola 2), respectivamente, totalizando 341 crianças, de 6 a 10 anos de idade. O Índice de Massa Corporal foi utilizado para mensurar o estado nutricional. O conhecimento sobre nutrição e alimentação foi avaliado por meio de um questionário de conhecimento, aplicado pré e pós intervenção. A intervenção nutricional constou de 12 aulas de 50 minutos cada. RESULTADOS: A melhora no conhecimento sobre alimentação e nutrição entre os estágios de pré e pós intervenção foi estatisticamente significante. CONCLUSÃO: O Programa "Nutriamigos®" atingiu o objetivo proposto, comprovando que é efetivo em escolas públicas e particulares e não deve ser diferenciado para meninos ou meninas, ou para crianças com peso normal ou excessivo.

Abstract The purpose of this study was to examine potential correlates of body fat (BF) and waist circumference (WC) in children. The sample included 328 children (169 boys) aged 9-11 years. BF (%) was measured using a bioelectrical impedance scale. WC measurements were made on exposed skin at the end of a normal expiration using a non-elastic anthropometric tape. Moderate-to-vigorous physical activity (MVPA) and sedentary behavior were measured using accelerometers. Participants with complete individual, family and home, and school environmental data were included in the analysis. Children averaged 21.3% in boys and 25.8% in girls for BF and 68.0 cm in boys and 67.2 cm in girls for WC. There was higher BF among girls (p<0.001), but no significant sex differences with respect to WC. In boys, breakfast consumption, bad sleep quality, and MVPA were associated with BF. Among girls, the only variables associated with BF were breakfast consumption and bad sleep quantity. Bad sleep quality and MVPA were associated with WC in boys. Among girls, WC was associated with breakfast consumption and bad sleep quantity. We identified correlates of BF and WC in children; however, few correlates were common for both BF and WC, and for both boys and girls.


Resumo O objetivo deste estudo foi examinar os potencias correlatos da gordura corporal (GC) e da circunferência da cintura (CC) em crianças. A amostra incluiu 328 crianças (169 meninos) de 9-11 anos. A GC (%) foi avaliada usando a bioimpedância elétrica e a CC (cm) usando uma fita antropométrica não elástica. A atividade física de moderada à vigorosa (AFMV) e o tempo sedentário (TS) foram mensurados usando acelerômetros. Os participantes que tinham informações completas individuais, familiares, e de ambiente da moradia e escolar foram incluídos nas análises. As médias das crianças foram 21,3% nos meninos e 25,8% nas meninas para GC e 68,0 cm nos meninos e 67,2 cm nas meninas para CC. A média das meninas foi maior do que nos meninos para GC (p<0.001). Não encontramos diferença significativa entre os sexos para CC. Nos meninos, o consumo de café da manhã, qualidade de sono ruim e AFMV foram associados com GC. Nas meninas, as únicas variáveis associadas foram o consumo do café da manhã e qualidade de sono ruim. Qualidade de sono ruim e AFMV foram associadas com CC nos meninos. Nas meninas, CC foi associada significativamente com consumo de café da manhã e qualidade do sono ruim. Identificamos correlatos da GC e da CC em crianças, no entanto, poucos correlatos foram comuns para GC e CC e em ambos os sexos.

Introdução: O consumo excessivo de bebidas açucaradas na infância é uma preocupação no acompanhamento das dificuldades alimentares, visto que pode gerar inadequações do estado nutricional. Objetivo: Estimar o perfil do consumo de alimentos de consistência líquida em crianças com dificuldades alimentares atendidas em um centro especializado e, verificar a associação entre a quantidade ingerida e a contribuição energética de bebidas açucaradas e idade e índice de massa corporal por idade. Métodos: Estudo transversal com 119 crianças de 0 a 18 anos, realizado em ambulatório especializado no atendimento de dificuldades alimentares. Os dados foram coletados de prontuários e diário alimentar. Resultados: A maioria da amostra era do gênero masculino (66,1%), considerada eutrófica quanto ao IMC/idade (77,3%), com média de idade de 4,1 anos (± 3,25). Houve diferença estatisticamente significativa para ingestão total de sucos, sendo que os meninos tiveram maior média de ingestão. A média de contribuição energética tanto para o sexo masculino quanto para o feminino é maior em relação ao consumo de leite. No entanto, não houve diferenças estatisticamente significativas entre os resultados (p > 0,05). Foram encontradas correlações muito fracas ou fracas entre volume ingerido e contribuição energética de bebidas açucaradas e idade e índice de massa corporal por idade. Conclusão: Crianças com dificuldades alimentares apresentam consumo elevado de bebidas açucaradas, havendo maior ingestão de sucos por crianças do sexo masculino e o leite foi o líquido que apresentou maior contribuição energética na dieta total deles. As associações encontradas entre volume consumido e contribuição energética de bebidas, idade e índice de massa corporal são fracas ou muito fracas não correspondendo à correlação entre as mesmas.


Introduction: Excessive consumption of sugar-sweetened beverages during childhood is a problem in the follow-up of children with feeding difficulties, as it can lead to inadequate nutritional status. Objective: To estimate the food consumption profile of liquid consistency in children with feeding difficulties attended at a specialized center in order to verify consumed amount and energy contribution of sugar-sweetened beverages and age and body mass index by age. Methods: A cross-sectional study was carried out with 119 children aged 0 to 18 years at an outpatient clinic specialized in feeding difficulties. Data were collected from medical records and dietary records. Results: Most of the sample was male (66.1%) and considered eutrophic for BMI/age (77.3%), with a mean age of 4.1 years (± 3,25). There was a statistically significant difference regarding the total amount of juice intake, and boys had a higher mean intake. Mean energy intakes for both males and females are higher in relation to milk consumption. However, there were no statistically significant differences between the results (p> 0.05). Weak correlations were found between ingested volume and energy contribution of sugar-sweetened beverages and age and body mass index by age. Conclusion: Children with feeding difficulties have high consumption of sugar-sweetened beverages, with a higher juice intake by male children. Milk was the liquid that presented the highest energy contribution in their total diet. The associations found between the volume consumed and the energy contributions of beverages, age, and body mass index were weak or very weak, not corresponding to the correlation between them.

The high consumption of added sugars in the diet of the Argentine population and its consequent effect on health are current concerns both at the clinical and public health levels. The objective of this study was to determine the main sources of added sugars in the Argentine diet and the proportional energy contribution of each of them. The sample consisted of 1266 individuals, representative of the country's urban population from 15 to 65 years old, stratified by region, age, gender and socio-economic level. Two 24-hour dietary recalls and one socio-economic questionnaire were carried out. Among the total food and beverages consumed by Argentines, 26.9% of added sugars were provided by soft drinks and 23.8% by infusions. The third place, with 15.4%, came from baked goods (bread, cookies, etc.) and the fourth, with 12%, from ready-to-prepare juices, ahead of sweets and candies and dairy products. Men, compared to women, consumed significantly more added sugars in soft drinks (32.6 vs. 22.1%), while women consumed more added sugars in infusions (25.5 vs. 21.8%), baked goods, sugar and honey. The lower income population consumed significantly more sugar in infusions at the expense of "mate" (21.4 vs. 7 g/day), while no differences were observed in the consumption of soft drinks by socio-economic level (32.9 vs. 34.4 g/day). Urgent measures based on education could improve the consumption habits of added sugars and the health of the population.


El alto consumo de azúcares añadidos en la dieta de la población argentina y su consecuente efecto sobre la salud son preocupaciones actuales tanto a nivel clínico como de salud pública. SUMMARY BACKGROUND: Epidemiological studies with dietary variables are complex methodologically, being the researcher responsible for anticipating, controlling, reducing and preventing methodological errors. Obesity accounts for almost one-third of the world's population and has consequences for childhood and adolescence. Multifactorial disorder must be faced in several aspects, being food and physical activity, modifiable risk factors. The EBANS aims to perform a diagnosis of the nutritional status of the Brazilian population from 15 to 65 years old, from all regions, and the parameters associated with obesity, with several possibilities of correlating data. METHODOLOGICAL PROCESS: Part of the ELANS study (n = 9218), the EBANS (n = 2000) has a weighted sample and data collection that allows: to evaluate the socioeconomic level of the population; perform a diagnosis of nutritional status (through anthropometric variables); to evaluate food intake (R24h and FFQ for beverages); and evaluate physical activity practice (IPAQ-long and accelerometer). METHODOLOGICAL OPPORTUNITIES: With national coverage, EBANS has the potential to compose regional analyzes, portray the current nutritional epidemiological condition, food consumption and physical activity pattern of the Brazilian population, at different life stages, and may have their data analyzed together or stratified, offering useful subsidies for the formulation of public policies. METHODOLOGICAL CHALLENGES: Each methodological step was designed to reduce errors and biases related to methodological challenges. CAAE REGISTRATION: 31670314.8.0000.5567. FINAL CONSIDERATIONS: Of great potential for future data analysis, EBANS tries to contribute to the generation of knowledge to foment policies and actions capable of changing the current obesity scenario.


RESUMO INTRODUÇÃO: Estudos epidemiológicos com variáveis dietéticas são complexos metodologicamente, e o pesquisador é responsável por antecipar, controlar, reduzir e prevenir erros metodológicos. A obesidade representa quase 1/3 da população mundial e agrega consequências que são observadas na infância e na adolescência. Desordem multifatorial deve ser enfrentada sob diversos aspectos, sendo alimentação e atividade física fatores de risco modificáveis. O EBANS se propõe a realizar um diagnóstico do estado nutricional da população brasileira de 15 a 65 anos, de todas as regiões, e dos parâmetros associados à obesidade, com diversas possibilidades de correlacionar dados. PROCESSO METODOLÓGICO: Parte do estudo ELANS (n=9218), o EBANS (n=2000) tem amostra ponderada e coleta de dados que permite: avaliar o nível socioeconômico da população; realizar diagnóstico do estado nutricional (por meio de variáveis antropométricas); avaliar consumo alimentar (R24h e QFA para bebidas); e avaliar prática de atividade física (IPAQ-longo e acelerômetro). OPORTUNIDADES METODOLÓGICAS: De abrangência nacional, o EBANS tem potencial para compor análises regionais, retratar a atual condição epidemiológica nutricional, de consumo alimentar e padrão de atividade física da população brasileira, em diferentes estágios da vida, podendo ter seus dados analisados em conjunto ou estratificados, oferecendo subsídios úteis para a formulação de políticas públicas. ABSTRACT This study aimed at describing the lipid profile of children with feeding difficulties (FD), as well as to verify the impact of clinical types of FD and other markers on the presence of dyslipidemias (DLP). It was a cross-sectional study with 61 children between 2 and 10 years old. The following data was collected from medical records: age, gender, duration of exclusive breastfeeding (months), dosages of total cholesterol, HDL, LDL, VLDL and triglycerides (according to recommendations for age), type of FD, BMI z-score, dietary intake of carbohydrates and lipids (% energy intake), and daily consumption of milk (ml), fiber (g) and sugar sweetened beverages (SSB, ml). T-Student-test and ANOVA test were used, with a 5% significance level. Children were mostly picky eaters (55.7%), and 47.5% had dyslipidemia, mostly low HDL-c (27.6%) and hypertriglyceridemia (21.9%). No significant relationship was found between DLP and duration of exclusive breastfeeding (p= 0.93), BMI (p> 0.40), type of FD (p> 0.26), or dietary characteristics (p> 0.12). Children with dyslipidemia tended to drink higher volumes of SSB when compared to recommended values (p= 0.044). The prevalence of DLP found was higher than the average shown in children. More studies are needed to prove if there is a true association between FD and dyslipidemia.


RESUMEN El objetivo de este estudio fue describir el perfil lipídico de niños con dificultades alimentares (DA) y verificar el impacto de los tipos clínicos de DA y otros marcadores sobre la presencia de dislipemias (DLP). Se trató de un estudio transversal con 61 niños de 2 a 10 años. Se obtuvieron los datos de edad, sexo, duración de la lactancia materna exclusiva (meses), colesterol total, HDL, LDL, VLDL y triglicéridos (según las recomendaciones para la edad), tipo de DA, índice-z del IMC, consumo dietético de carbohidratos y lípidos (% energético), consumo diario de leche (ml), fibra (g) y bebidas endulzadas (SSB, ml). Se usaron pruebas T-Student y ANOVA, con nivel de significancia del 5%. Los niños fueron selectivos (55,7%) y el 47,5% dislipidémicos, principalmente con HDL-c bajo (27,6%) y hipertrigliceridemia (21,9%). No se encontró relación significante entre DLP y la duración de la lactancia materna exclusiva (p= 0,93), el IMC (p> 0,40), el tipo de DA (p> 0,26) o las características dietéticas (p> 0,12). Los niños dislipidémicos tendieron a beber mayores volúmenes de SSB en comparación con los valores recomendados (p= 0,044). La prevalencia de DLP encontrada es más alta que el promedio que se muestra en niños. Se necesitan más estudios para demostrar asociaciones sólidas entre DA y dislipidemia.

Abstract Objective: To identify factors associated with total sedentary time and screen time in children aged 9-11 years. Methods: For seven consecutive days, 328 children (51.5% boys) used accelerometers to monitor total sedentary time. Screen time was calculated by the self-reporting method. Individual, family, family environment, and school environment questionnaires were filled out. Body composition was measured using a Tanita scale. Results: The mean sedentary time was 500 min/day (boys: 489, girls: 511, p = 0.005), and mean screen time was 234 min/day (boys: 246, girls: 222, p = 0.053). In both genders, factors associated with sedentary time were healthy dietary pattern and moderate-to-vigorous physical activity. In boys, only moderate-to-vigorous physical activity was significant; in girls, the healthy dietary pattern, moderate-to-vigorous physical activity, and transportation to school were significant. As for the screen time, the associated factors were body mass index and healthy dietary pattern (both genders). In boys, the associated factors were body mass index, healthy dietary pattern, and television in the bedroom. In girls, the associated factors were healthy dietary pattern, transportation to school, and physical activity policies or practice at school. Conclusion: Several associated factors were identified in the association between total sedentary time and screen time in children; however, only the healthy dietary pattern was common between sedentary time and screen time.


Resumo Objetivo: Identificar os fatores associados do tempo sedentário total e tempo de tela em crianças entre 9-11 anos. Métodos: Durante sete dias consecutivos, 328 crianças (51,5% meninos) usaram acelerômetros para monitorar o tempo sedentário total. O tempo de tela foi calculado pelo método autorreferido. Questionários com fatores individuais, familiares, ambiente familiar e escolar foram preenchidos. A composição corporal foi medida usando uma balança Tanita. Resultados: A média do tempo sedentário (min/dia) foi de 500 (meninos: 489; meninas: 511; p = 0,005), e do tempo de tela foi 234 (meninos: 246; meninas: 222; p = 0,053). Em ambos os sexos, os fatores associados do tempo sedentário foram o padrão de dieta saudável e atividade física de moderada a vigorosa. Nos meninos, apenas a atividade física de moderada a vigorosa foi significativa e nas meninas o padrão de dieta saudável, atividade física de moderada a vigorosa e transporte para a escola foi significativo. Já para o tempo de tela, os fatores associados foram o índice de massa corporal e padrão de dieta saudável (ambos os sexos). Nos meninos, os fatores associados foram o índice de massa corporal, o padrão de dieta saudável e televisão no quarto. Nas meninas, os fatores associados foram o padrão de dieta saudável, transporte para escola e políticas ou práticas de atividade física na escola. Conclusão: Vários fatores associados foram identificados entre tempo sedentário total e tempo de tela em crianças. Entretanto, somente o padrão de dieta saudável foi comum entre tempo sedentário e tempo de tela.

RESUMO Objetivo: Elaborar pontos de corte de circunferência da cintura de acordo com o estadiamento puberal para identificar sobrepeso em adolescentes. Métodos: Trata-se de um estudo longitudinal, aprovado pelo Comitê de Ética em Pesquisa, com 557 adolescentes, de 10 a 15 anos de idade, selecionados em escolas públicas. Aferiram-se as medidas de circunferência de cintura, braço, pescoço e quadril, percentual de gordura, massa corporal, estatura e pressão arterial. Para determinar a maturação sexual, foi utilizada a autoavaliação da escala de Tanner. Aplicou-se a curva ROC (Receiver Operating Characteristic Curve) para determinar: poder preditivo, sensibilidade, especificidade e os pontos de corte de circunferência de cintura para identificação de sobrepeso. Resultados: Observou-se correlação positiva entre circunferência de cintura e massa corporal, índice de massa corporal (IMC), circunferência do braço e quadril, razão cintura/quadril e pressão arterial em ambos os sexos. Os pontos de corte para circunferência de cintura de acordo com o estadiamento puberal para identificação de sobrepeso que apresentaram maior desempenho na curva ROC foram: 71,65 cm para meninas pré-púberes, 67,90 cm para meninas púberes, 70,25 cm para meninas pós-púberes e 66,45 cm para meninos púberes. Faixa etária, massa corporal, estatura, IMC, porcentagem de gordura, circunferência do braço e do quadril foram considerados fatores preditores da circunferência da cintura alterada. Conclusões: Os pontos de corte de circunferência da cintura de acordo com o estadiamento puberal demonstraram excelente desempenho para a identificação de sobrepeso, podendo ser considerados fidedignos para a população de adolescentes brasileiros, uma vez que utilizar apenas a idade cronológica na adolescência pode subestimar o estado nutricional.


ABSTRACT Objective: To establish waist circumference cut off points according to pubertal staging to identify overweight in adolescents. Methods: Longitudinal study approved by the Ethics Research Committee and conducted with 557 adolescents, aged 10 to 15 years old, selected from public schools. Waist, arm, neck and hip circumferences, body fat percentage, body mass index (BMI), height and blood pressure were measured. Pubertal staging was evaluated by Tanner self assessment scale. The Receiver Operating Characteristic Curve (ROC curve) was used to determine predictive power, sensitivity, specificity and waist circumference cut off points to detect overweight. Results: There was a positive correlation between waist circumference and weight, BMI, upper arm and hip circumferences, waist-to-hip ratio and blood pressure in both sexes. Cut off points for waist circumference according to pubertal stage as related to overweight in adolescents with the best performances in ROC curve were: 71.65 cm for prepubescent girls, 67.90 cm for pubescent girls, 70.25 cm for post pubescent girls, and 66.45 cm for pubescent boys. Age, weight, height, BMI, body fat percentage, arm and hip circumferences were associated to altered waist circumference. Conclusions: The establishment of cut off points for waist circumference according to pubertal staging was proven a good means to identify overweight. These cut off points can be considered reliable for the Brazilian adolescent population, as the isolated use of chronological age in adolescents may underestimate their nutritional status.

RESUMO Objetivo: Revisar evidências atuais da relação entre transtorno obsessivo compulsivo e dificuldades alimentares. Métodos: Revisão das bases Science Direct e PubMed no período entre 2007 e 2017 em inglês, português e espanhol, com os termos em associação "transtorno obsessivo compulsivo" e "picky eating/dificuldade alimentar". Foram selecionados apenas estudos de coorte, caso controle ou transversal, realizados em qualquer país, com crianças, adolescentes e/ou adultos e de qualquer tamanho amostral. Foram excluídos os artigos de opinião. Resultados: Cerca de 245 artigos foram selecionados e apenas 4 foram incluídos no estudo, segundo critérios de seleção. Os trabalhos descrevem essencialmente que há diferença no comportamento seletivo entre os sujeitos com e sem transtorno obsessivo compulsivo, com tendência para exacerbação de sintomas como nojo, ansiedade e escore de inflexibilidade de comportamento alimentar nos pacientes com esse transtorno. Conclusões: Existem sintomas compartilhados entre transtorno obsessivo compulsivo e dificuldade alimentar. O estudo alerta aos profissionais que acompanham pacientes com dificuldades alimentares para a importância da investigação de possíveis comorbidades psiquiátricas.


ABSTRACT Objective: To review current evidence on the relationship between obsessive-compulsive disorder and feeding difficulties. Methods: Review the Science Direct and PubMed databases between 2007 and 2017 in English, Portuguese and Spanish. The search terms, used in association, were "obsessive compulsive disorder" and "picky eating/feeding difficulty". Cohort, case control and cross sectional studies were included that analyzed children, adolescents and/or adults of any sample size from any country in the world. Opinion articles were excluded. Results: Around 245 articles were selected, and only 4 were included in this review, according to previous criteria. Results from the studies essentially described that there is indeed a difference in "picky" behaviors between subjects with and without obsessive-compulsive disorder. Patients with obsessive-compulsive disorder tend to have exacerbated symptoms of disgust, anxiety and a higher eating behavior inflexibility score. Conclusions: Obsessive-compulsive disorder and feeding difficulties patients share common symptoms. The present study alerts health professionals who follow patients with feeding difficulties as to the importance of investigating possible psychiatric comorbidities.

Abstract Objective: To estimate the prevalence of breakfast consumption and describe the foods and nutrients consumed at this meal and throughout the day by Brazilian adolescents. Method: A total of 7276 adolescents aged 10-19 years were evaluated in the Brazilian National Dietary Survey 2008-9, a population-based cross-sectional study. Individuals' information was collected at home. Dietary data were obtained by two food records. Breakfast was defined as the first eating occasion of the day that occurred between 6 and 9:59 am. Differences between breakfast consumers, occasional consumers, and skippers were tested through Pearson's chi-squared test or F-test of regression analysis. Results: Breakfast was consumed by 93% of adolescents and it was associated with age, income, geographic region and household area. The most frequently consumed foods at breakfast were white bread, coffee, butter/margarine, refined cookies and crackers, and whole milk. The mean daily intakes of total energy, sugar, and calcium were higher among occasional consumers and skippers. Breakfast consumers had higher intake of vitamins B12, C, and D. Breakfast contributed more to total intake of calcium, phosphorus, thiamin, riboflavin, and vitamins A, B6, and D (17-32%), trans fat and sodium (about 30%) and less to folate, vitamin C, iron, zinc, and fiber (8-12%) and energy intake (16%). Conclusions: Although the prevalence of breakfast consumption among Brazilian adolescents was high, the overall nutritional quality of this meal is suboptimal, highlighting the need to support adolescents and their families to make more nutrient-dense food choices.


Resumo Objetivo: Estimar a prevalência de consumo de café da manhã e descrever os alimentos e nutrientes consumidos nessa refeição e durante todo o dia por adolescentes brasileiros. Método: Foram avaliados 7.276 adolescentes entre 10-19 anos no Inquérito Nacional de Alimentação 2008-2009, um estudo transversal de base populacional. As informações dos indivíduos foram coletadas nos domicílios. Os dados alimentares foram obtidos por dois registros alimentares. O café da manhã foi definido como a primeira refeição do dia entre 6h e 9h59. As diferenças entre os consumidores de café da manhã, consumidores ocasionais e aqueles que não tomam café da manhã foram testadas por meio do teste de qui-quadrado ou teste F para análise de regressão. Resultados: O café da manhã foi consumido por 93% dos adolescentes e seu consumo foi associado à idade, renda, região geográfica e área residencial. Os alimentos mais frequentemente consumidos foram pão branco, café, manteiga/margarina, biscoitos e bolachas refinados e leite integral. A ingestão média diária de energia total, açúcar e cálcio foi maior entre os consumidores ocasionais e aqueles que não tomam café da manhã. Aqueles que tomam café da manhã apresentam maior ingestão de vitaminas B12, C e D. O café da manhã contribuiu mais para a ingestão total de cálcio, fósforo, tiamina, riboflavina e vitaminas A, B6 e D (17%-32%), gordura trans e sódio (cerca de 30%) e menos para a ingestão de ácido fólico, vitamina C, ferro, zinco e fibras (8%-12%) e energia (16%). Conclusões: Apesar de a prevalência de consumo de café da manhã entre os adolescentes brasileiros ter sido alta, a qualidade nutricional dessa refeição está abaixo do ideal, destaca a necessidade de apoio aos adolescentes e suas famílias para possibilitar a escolha de alimentos mais densos em nutrientes.

ABSTRACT

Background: Iron deficiency anemia and feeding difficulties (FD) are common issues in childhood, reinforcing the concern about the risk of micronutrient deficiencies. FD do not necessarily reflect nutritional deficiencies, since they may or may not relate to specific nutrient sources. The objective of the study is to describe the prevalence of iron depletion and iron deficiency anemia in children with FD and to seek associations with diagnosis and its markers. Methods: This is a cross-sectional study with 68 patients (convenience sample). The following data were assessed through medical records: age (months), gender, exclusive breastfeeding duration (months), birth weight (kg), iron supplementation, hemoglobin (Hb), ferritin, and C-reactive protein (CRP) levels, repertory of foods consumed (food inventory and 3-day food record analysis), and diagnosis of FD. Data were classified according to references for age and were analyzed using correlation tests, Student's t test, ANOVA and chi-square test, or its nonparametric equivalents. A significance level of 5% was considered. Results: Iron depletion and anemia were identified in 10.1 and 6% of children, respectively. Picky eating was diagnosed in 35.3%. Food repertory consisted on average of 21 foods, with null correlation to Hb and ferritin. The average fortified milk intake was 517 ml/day, with null correlation to Hb. There was no effect of diagnosis of FD on Hb (p = 0.18) or ferritin (p = 0.52). The same was verified in the children without supplementation, to both Hb (p = 0. 54) and ferritin (p = 0.08). Conclusions: No evidence of association between diagnosis of FD or repertory of foods to anemia or iron depletion was found, which could be a reassuring factor for caregivers. Reproduction in large scale as well as inclusion of dietary intake variables is suggested for further research.

Abstract Objective: This study aimed to identify the prevalence and factors associated with body mass index (BMI) in children aged 9-11 years. Methods: The study is part of the International Study of Childhood Obesity Lifestyle and the Environment (ISCOLE). Body composition was determined using the bipolar bioimpedance technique. The mean BMI value was categorized as recommended by the World Health Organization. For seven consecutive days, participants used an accelerometer to objectively monitor sedentary behavior (SB) and moderate to vigorous physical activity. Individual factors (anthropometric and behavioral), family aspects, and family and school environment were provided by participants and parents and were analyzed by multilevel linear regression adjusted for gender, ethnicity, school, number of siblings, and total annual family income. Results: The mean BMI was 20.1 kg/m2, and 51.8% of the children were overweight/obese (50.3% boys, 53.4% girls, p = 0.014). Considering all participants, the associated factors of BMI were body fat percentage (BF%, β = 0.0216, p < 0.001) and screen time (ST, β = 0.0050, p = 0.006). In boys, the associated factors were BF% (β = 0.0209, p < 0.001), ST (β = 0.006, p = 0.036), and healthy eating policies or practices (β = 0.0276, p = 0.025). In girls, only BF% was associated (β = 0.0221, p < 0.001) with BMI. Conclusions: High prevalence of overweight/obesity was observed in children from São Caetano do Sul. Different associated factors were identified between the genders, with only BF% being common in both genders.


Resumo Objetivo: Identificar a prevalência e os fatores associados do índice de massa corporal (IMC) em crianças de 9-11 anos. Métodos: O estudo faz parte do International Study of Childhood Obesity Lifestyle and the Environment (Iscole). A composição corporal foi determinada pelo método da bioimpedância bipolar. O valor médio do IMC foi categorizado conforme sugerido pela Organização Mundial de Saúde. Durante sete dias consecutivos, os participantes usaram acelerômetro para monitorar objetivamente o comportamento sedentário (CS) e atividade física de moderada a vigorosa. Fatores associados individuais (antropométricos e comportamentais), aspectos familiares, ambiente familiar e escolar foram preenchidos pelos participantes e pais e foram analisados pela regressão linear multinível ajustados para sexo, etnia, escola, número de irmãos e renda familiar total anual. Resultados: A média do IMC foi de 20,1 kg/m2, 51,8% das crianças estavam com excesso de peso/obesidade (50,3% meninos; 53,4% meninas; p = 0,014). Considerando todos os participantes, os fatores associados do IMC foram a porcentagem de gordura corporal (%GC; β = 0,0216; p < 0,001) e o tempo de tela (TT; β = 0,0050; p = 0,006). Nos meninos, os fatores associados foram a %GC (β = 0,0209; p < 0,001), TT (β = 0,006; p = 0,036) e políticas ou práticas de alimentação saudável (β = 0,0276; p = 0,025). Já nas meninas, somente a %GC foi associada (β = 0,0221; p < 0,001) com o IMC. Conclusões: Mostramos valores elevados de prevalência de excesso de peso/obesidade em crianças de São Caetano do Sul. Diferentes fatores associados foram identificados entre os sexos, somente a %GC foi comum em ambos os sexos.

RESUMO Objetivo Adaptar a escala de silhuetas originais em cartões de papel e validar para uso digital. Métodos Participaram dessa pesquisa 88 jovens, os quais foram avaliados quanto ao índice de massa corporal (IMC) e questões sobre a imagem corporal com as escalas de silhuetas de papel e digital em relação ao corpo atual (IMC-A) e o corpo desejado (IMC-D). O processo de validação da escala digital se dá pela comparação entre o IMC com o IMC-A. Resultados Os resultados foram satisfatórios (0,70) para o grupo quando avaliado em conjunto e para o sexo masculino. No entanto, para o sexo feminino obtivemos um valor relativamente baixo, na concordância de Lin (0,64), mas com valores de ICC (0,89) e correlação de Spearman (0,85) aceitáveis. Conclusão As imagens no computador podem ser utilizadas para a avaliação da imagem corporal de jovens. Porém, é mais seguro realizar associações com questões referentes à prática de atividade física e do comportamento alimentar que, em conjunto, possam auxiliar na avaliação da insatisfação corporal.


ABSTRACT Objective Adapt The Brazilian Silhouette Scale, rendering them fit for digital use. Methods Eighty-eight young people took part in the survey, in which they were assessed regarding their body mass index (BMI) and topics related to body image, with the use of paper and digital silhouettes in relation to the current body (BMI-C) and the desired body (BMI-D). The process used to validate the digital scale is done by comparing the BMI with BMI-C. Results Were satisfactory (0.70) for the group, as a whole and for the males included. However, females showed a relatively low value at Lin's concordance (0.64), but with acceptable ICC (0.89) and Spearman (0.85) values. Conclusion The computer images can be used to assess body image for young people. However, it is safer to make associations with topics regarding physical activity and eating behavior, which together can help us assess body dissatisfaction.
